New Business Institute Resource and Blog Post on Age-Friendly Health Systems

A new blog post by Dr. Terry Fulmer, President of The John A. Hartford Foundation, and resource guide from n4a's Aging and Disability Business Institute showcase The John A. Hartford Foundation's Age-Friendly Health Systems initiative and how community-based organizations, such as AAAs, can use the model to improve the quality of care for older adults and optimize value for health systems.

Launched in 2017, the Age-Friendly Health Systems initiative has sparked innovation and improvement across the country, and has enabled AAAs and other CBOs to develop and deepen collaborations with clinical partners. Together, these collaborations can ensure that older adults receive the best care possible across all long-term care settings.

In partnership with the Evidence-Based Leadership Council, the Business Institute has also released a resource guide that offers an overview of the the "4 Ms" (What Matters, Medication, Mentation and Mobility) that are the cornerstones of the Age-Friendly Health Systems initiativeAs AAAs increasingly look to partner and contract with health systems in their communities, this guide can help your agency think about the value that it and the evidence-based health promotion programs your agency offers can bring to health systems and the patients they serve.
